The practical answer

Give employees one employer contact who can identify the form, reporting year and responsible issuer. Retrieve the correct approved statement, investigate genuinely missing records, and document the response. A request for another copy must not trigger another original IRS filing.

This guide is for HR and benefits teams serving employees after ACA statement preparation. The business owns the request queue, identity verification and response record. Employees should not have to determine which payroll vendor, carrier or administrator held a particular extract.

The process uses the 2025 B-series and C-series instructions, together with Notice 2025-15 where the organization uses the alternative manner of furnishing. It complements the employer's filing calendar without replacing it.

Record the exact request without collecting unnecessary data

Capture the date received, reporting year, requested form if known, internal employee identifier and the reason for the request. Distinguish a missing first copy, a reprint, an inaccessible electronic statement and a suspected data error. These cases can look similar in an inbox but require different responses.

Verify the requester through the employer's established process before disclosing information. Avoid collecting an entire household tax file to locate one statement. Keep sensitive identifiers in the approved employee system and use a case reference in the service worksheet. Record how the requester can be reached if additional facts are needed.

Find the responsible reporting organization

For employer Form 1095-C information, identify the ALE member and its reporting records. For a Form 1095-B concerning insured coverage, the carrier may be the responsible issuer. The employer can help route the inquiry without preparing an unauthorized replacement for another organization's statement.

An individual Marketplace Form 1095-A ordinarily comes from the Marketplace. An employee request for that document does not justify generating another employer form. Explain which reporting relationship the employer can resolve and provide the appropriate verified contact route for the remaining request. Retain a clear handoff rather than simply marking the case outside scope.

Apply the employer's furnishing method and response clock

Where the organization uses the alternative manner of furnishing under Notice 2025-15, it must satisfy the applicable website notice requirements and provide requested copies on time. The notice describes timeliness as the later of January 31 of the following year or 30 days after the request. Track the actual receipt date and applicable requirements rather than relying on a generic support response target.

Also account for the required notice posting and retention, permitted delivery methods and any separate state obligations. The alternative furnishing method does not eliminate the employer's IRS filing requirement. Assign an owner for aged requests and escalate cases early when source investigation threatens timely delivery.

In this fictional queue, only the third case suggests an unresolved employer reporting record. The first request is a furnishing task, not another original transmission. The second requires a useful issuer handoff. Record each result with its evidence so a later team member can understand why the actions differ.

If the third case confirms an omitted return, follow the applicable original filing and furnishing procedures. Do not mark a new return corrected solely because the employee made a service request.

Close the request using delivery and reporting evidence

Record the statement version, delivery date and approved delivery method, or the documented issuer handoff and follow-up decision. If the request exposed an error or omission, link it to the reporting case and keep the service request visible until the promised response is delivered.

Review unresolved and repeated requests after the furnishing period. Several employees unable to locate copies can indicate a communications problem, portal access issue or missing release, rather than several unrelated employee mistakes. Use that pattern to improve the business process and preserve the evidence needed for next year's service team.

Common questions

Should a reprint create another original IRS return?

No. Retrieving and furnishing an appropriate copy is a separate task from agency filing. Check the existing approved record and version, and investigate any actual filing issue separately.

Can we ignore a former employee's request?

Use the relevant reporting year and employer population to investigate it. Current employment status does not determine whether the employer prepared or was required to prepare a statement for an earlier period.

Does website notice remove the need to answer requests?

No. The alternative manner of furnishing includes timely response obligations and notice requirements. Track receipt dates, applicable response deadlines and delivery evidence under the guidance the employer relies on.

Should HR create a replacement 1095-A?

No. Individual Marketplace coverage is ordinarily reported by the Marketplace. Help the employee reach the responsible issuer while separately resolving any employer reporting or furnishing task.

What if the employee was accidentally omitted from the filing?

Escalate the underlying reporting issue using the approved population and source records. Determine the required original filing or other action under the applicable instructions, and track the employee response without treating an omission as just a portal problem.
